Incident Report Submission

An Incident Report must be completed for any incident, accident, injury, misconduct, or safeguarding concern that occurs during training, matches, tournaments, or any event associated with Glasnevin Schoolboys FC.

Please ensure that this report is submitted to the Club Secretary within 48 hours of the incident. When completing the form, provide a clear and detailed description of what occurred, including the people involved and any actions taken at the time.

Where available, supporting documentation such as referee reports, injury reports, witness statements, photographs, or other relevant information should be attached to assist with the club's review of the incident.
